3. Check or Money Order in the amount of $100.00 U.S. Funds. We will not waste our time at even reading your book until your check clears the bank.

4. Your Name and Address so we can contact you if we have any questions. (telephone number and e-mail address is also helpful)

5. Any additional comments or things you feel should be included.
